Sales Tax Exemption Certificate

To apply for a sales tax exemption, download the Texas Sales Tax Exemption Certificate and fax the completed form to (505) 241-2371.

It is critical to include the following information on the Texas Sales Tax Exemption Certificate:

  • Name of company as reflected on your agreement
  • Company account number or ESI ID number (If multiple account numbers, all accounts must be listed)
  • Company phone number
  • Service address
  • Dates of requested refund (State Comptroller statute is four years from today’s date)
  • Customer’s signature or authorized employee of the customer.

Make sure to mark the appropriate box for the exemption that is being claimed. If you are claiming a manufacturing, processing, fabricating or other non-commercial purpose exemption and the electricity used is less than 100 percent, First Choice Power needs the certifying engineer’s signature or stamp affixed to the form with the percentage used noted. A copy of the certifying engineer’s signature and percentage page from the Predominate Use Study is also acceptable. The Sales Tax Exemption Certificate does not exempt customers from the Gross Receipts Tax or the PUC Assessment.
